Epinephrine administration report form
Use this form any time epinephrine is administered in your school.
Medication error reports
Please also email Caitlin Pettengill, DNP, RN to report a medication error in school that caused harm or required assessment.
Immunization Surveys
The Massachusetts Immunization Information System (MIIS) is a web-based immunization registry. 
 
Your school immunization surveys, generally due in late January, are done through MIIS. More information and training materials can be found at the links below:
CSHS Monthly Report

SBIRT
All public school districts with a middle or high school are responsible for submitting this report.
(CSHS-funded districts should include these data with their Status Report, not as an additional data submission).

BMI Report
All public school districts with grades 1, 4, 7, or 10 are responsible for submitting this report.
