Epinephrine administration report form | Use this form any time epinephrine is administered in your school. |
Medication error reports | Please also email Caitlin Pettengill, DNP, RN to report a medication error in school that caused harm or required assessment. |
Immunization Surveys | The Massachusetts Immunization Information System (MIIS) is a web-based immunization registry. Your school immunization surveys, generally due in late January, are done through MIIS. More information and training materials can be found at the links below: |
CSHS Monthly Report | All CSHS-funded districts and non-public schools are responsible for submitting this report. For more info on School Health & Workforce Development Grant data collection tools, see the MDPH School Health Services Resource Page. |
