Learning Objectives:

1. Review hazards of hospitalization to older adults and the role geriatricians play in managing their care 

2. Discuss RUSH's Geriatric Medicine inpatient START and TOC consults, focusing on an interdisciplinary transition of care model 

3. Describe improved transitions of care hand off process utilizing a HIPAA-compliant phone app  

4. Discuss the benefits and challenges of utilizing the geriatric consult team to address multidisciplinary needs of patient facing complex transitions of care 

5. Explore ways to implement elements of these interventions in other health systems
