  • Decide when it is appropriate to refer patients to Community Partners (CPs) in relation to the other WellSense Care Management programs
  • Identify which CP organizations they can refer to and how they can refer
  • Know how to reach the CPs
  • Complete what is expected of their practice to fulfill MassHealth’s Care Team requirement
