This ECHO® series is designed for PCPs, APPs, Pharmacists, Cardiologists, Nephrologists and Nurses. Teams and individual providers are welcome to attend!

 

Learning Objectives
At the conclusion of this learning activity, participants will be better able to:

  • Integrate guideline-directed CKD testing using eGFR and uACR testing for people with T2D, hypertension, or other risk factors for CKD
  • Determine appropriate care strategies for patients based on their risk categories for CKD progression, morbidity, and mortality
  • Individualize treatment recommendations to address the different components of cardiovascularrenal-metabolic conditions in patients
  • Identify ways to improve clinician collaboration related to the management of their patients with cardiovascular-renalmetabolic conditions
