Target Audience
This program is intended for primary care providers including internal and family medicine physicians, nurse practitioners, physician associates/physician assistants, nurses, registered dietitians, and pharmacists as well as population health managers and others involved in the care delivery of patients living with type 2 diabetes and obesity.
Learning Objectives
Upon completion of this activity, learners should be able to:
- Explain the importance of looking at data over time in QI
- Create a run chart to understand variation in your data (common cause and special cause) and determine if your improvement efforts are succeeding
