This event is run by Boston Children's Hospital and is an external course. Please contact them if you have any questions.

• 8am to 4pm both days
• Health assessment topics to include: GI, Orthopedics, Respiratory, Cardiology, Dermatology, ENT, Eye, Neurology, Dental and Mental Health 
• Nursing continuing education credits pending 
• Pre-registration is required
• Registration fees are non-refundable
• Purchase orders are accepted.  To register via purchase order, stop here and email name, email address and purchase order number to: judi.naar@childrens.harvard.edu  NO NEED TO SUBMIT THIS REGISTRATION FORM if registering via purchase order.  Purchase orders must be received at least 7 days prior to the start of the program in order for registration to be complete
• Group discounts are available, for every 5 registered from a district the 6th is free.  Please email judi.naar@childrens.harvard.edu  the names and email addresses of those in your group to register .  NO NEED TO SUBMIT THIS REGISTRATION FORM if registering with a group discount
• Breakfast, lunch and snacks provided both days of program
• Please let us know of any food allergies and/or special accommodations needed when registering below so that we may alert the conference center
