Alzheimer's Screening Quality Improvement Activity 2024-2025 - BMC General Internal Medicine
A PI-NCPD, PI-CME and Portfolio project in General Internal Medicine at Boston Medical Center to improve dementia screening and appropriate referrals to specialists for patients 65 and older provided by Boston University Chobanian and Avedisian School of Medicine, CCE.

Target Audience
Physicians, PAs, Nurses and others involved in the care of patients with dementia.
Learning Objectives
Upon completion of this activity, learners should be able to:
1. Complete at least 1 improvement cycle (PDSA) on a dementia screening and care related project of their choice with the guidance of faculty and ongoing mentorship.
2. Apply the knowledge, tools and experience to implement improvement projects independently after completion of the course.
3. Organize interprofessional teams and achieve sustainable improvements according to the goals of their department and the organization.
