Target Audience

Physicians, nurses, nurse practitioners and social workers

Learning Objectives

At the end of this activity, the learner or healthcare team should be able to:

  • Describe the current state of maternal/perinatal health in Massachusetts, including disparities, trends, and priority populations, using the latest DPH data. 
  • Identify the role of Community Health Centers (CHCs) in improving maternal health outcomes across the prenatal, perinatal, and postpartum continuum. 
  • Recognize evidence-based and emerging models of maternal care being implemented within Massachusetts CHCs. 
  • Assess opportunities for partnership between CHCs, birth centers, public health agencies, and community-based organizations. 
  • Evaluate innovative tools and approaches, including group care models and home monitoring technologies, to enhance access, engagement, and outcomes. 
  • Translate lessons learned into actionable strategies that participants can adapt and implement within their own health centers.
